History

Often called the "best kept secret of Contra Costa County", Contra Costa Health Plan (CCHP) has been serving the health needs for people in the County for 40 years. CCHP was the first federally qualified, state licensed, county sponsored HMO in the United States. In 1973, we became the first county sponsored health plan in California to offer Medi-cal Managed Care coverage and in 1976, we became the first county run HMO to serve Medicare beneficiaries. In the 1980's, we expanded our programs to reach out to county employees, businesses, individuals and families. In 2014 as part of the implementation of the Affordable Care Act, CCHP enrolled over 35,000 newly eligible individuals into Medi-Cal Managed Care and in 2015 over 25,000 more individuals enrolled into CCHP Medi-Cal Managed Care.

Contra Costa Health Plan provides managed care health insurance with its safety net community and county provider partners at an affordable price for diverse populations. We offer patient-centered medical homes to assure coordinated, comprehensive, compassionate and quality care.

History of Excellence

MILESTONES

In 1973 Contra Costa Health Plan (CCHP) began enrolling Medi-cal patients into our Health Plan. As the first County Sponsored Health Plan in California, we began a long tradition of ground breaking innovation. In the ensuing years, CCHP continued to be a model for health plans around the country as we expanded our membership to include Medicare beneficiaries, County Employees, Private Individuals and Families, Small and Large Business Groups.

In 1997 CCHP was designated the County's Local Initiative for Medi-cal Managed care and in 1998 we became the Community Provider for children eligible for the Healthy Families Program.

In 2001 CCHP's Advice Nurse Program received National Accreditation through URAQ, THE INDUSTRY LEADER IN MEDICAL MANAGEMENT and in 2006 CCHP was chosen as the lead agency for the Health Coverage Initiative, expanding Health Coverage to thousands of uninsured residents of Contra Costa County.

In 2008 CCHP celebrated 35 years and 80,000+ members.

In 2011 CCHP enrolled over 90% of Seniors and Persons with Disabilities who are mandated to choose a Managed Care Health Plan in Contra Costa County.

In 2014 as part of the implementation of the Affordable Care Act, CCHP enrolled over 35,000 newly eligible individuals into Medi-Cal Managed Care.

In 2015 as more individuals learned of eligibility for Medi-Cal under the Affordable Care Act, CCHP enrolled over 25,000 newly eligible individuals into Medi-Cal Managed Care.

CCHP now serves over 200,000 people and continues to be at the forefront of offering comprehensive, quality health coverage.

In February, we received our second three year Accreditation from NCQA (National Committee for Quality Assurance).
