Health Care for the Homeless


Health Care for the Homeless (HCH) provides high-quality health services to patients who are experiencing homelessness in Contra Costa County.

Our team includes bilingual medical, dental, behavioral health, social support and administrative professionals who provide direct care and referrals for many services.

Our clinics are open and free to all who are experiencing homelessness, regardless of insurance or immigration status.


Due to the Shelter-in-Place and the need for COVID-19 response, the Health Care for the Homeless schedule will be changing frequently, please call the HCH main line at 925-608-5300 for the most up to date information.

    • Mobile clinics (No appointment required. First come, first serve basis)
      • East County: Thursdays 1:00-3:30pm Antioch Shipyard, Tuesdays 8:30-12pm West Pittsburg Community Church
      • Central County: Fridays 9:00am-12:30pm Martinez Marina
      • West County: Mondays 8:00-3:00pm and Thursdays 8:00-11:00am at Bay Area Rescue Mission, Richmond
    • Ambulatory clinics Currently closed due to the Shelter-in-Place Public Health order.

  • Services

    Services provided at our clinics include:

    • Basic medical care
    • Tuberculosis testing and treatment
    • Free, confidential, on-site infectious disease testing including STD and health screenings (such as for diabetes)
    • Immunizations
    • Assistance with wound and medication management
    • Referral to mental health and substance use treatment
    • One-on-one behavioral health therapeutic case management and referrals as needed
    • Dental services (dental clinics only)
    • Enrollment help for health insurance (Medi-Cal)
    • Health education
    • Connection to doctors and providers for ongoing primary care
    • Outreach and community advocacy
    • Linkages to General Assistance, CalFresh and other benefits
    • Referrals to vision services

    Contra Costa Health Services advice nurse is available on a 24-hour basis, including weekends and holidays, if you need non-emergency medical advice or information. Call 1-877-661-6230 and press 1.

    Our mission is to improve the health of the homeless population in Contra Costa County by increasing access to health care with a team-oriented approach to health care that focuses on harm reduction and integration of behavioral and medical services.

    Our vision is that all persons experiencing homelessness in Contra Costa County can access comprehensive health care services to improve health status, effectively manage any chronic conditions and thrive as community members.


    Contra Costa Health Care for the Homeless was named a 2018 Health Center Quality Leader by the Health Resources and Services Administration, an agency of the U.S. Department of Health and Human Services.

    Respite Center

    The Philip Dorn Respite Center, located in Concord, is a respite care program for homeless adults who are discharging from local hospitals and require medical stabilization services. Respite care refers to recuperative services for those homeless persons who may not meet medical criteria for hospitalization, but who are too sick or medically vulnerable to reside in an emergency shelter and cannot be returned to the streets. Clients are ONLY admitted when discharged from a hospital; walk-ins are not accepted. This program is a joint effort between Health Care for the Homeless and the Division of Health, Housing & Homeless Services.

    The primary goal of this program and all emergency housing programs is to get homeless persons off the street and help them achieve their highest level of self-sufficiency.

    Criteria for Admissions
    • Until further notice, prior to admission all clients must be at least 10 days post COVID-19 vaccination.
    • Must be homeless or lack adequate housing to support recovery.
    • Must have a medical condition that can be effectively addressed within a limited amount of time, ≤ 6 weeks.
    • Must be ≥ 18 years old.
    • Must be willing and able to comply with C.C.C. Shelter and Medical Respite rules and agree to admission.
    • Must be able to perform all activities of daily living independently, including taking own medications.
    • Must be independently mobile and able to self-transfer in and out of bed.
    • Must be independent with wound care, or need assistance less than 4 times a week, or have Home Health nursing provided.
    • Must be continent of urine and stool.
    • Must not have received benzodiazepine for alcohol withdrawal in past 24 hours.
    • Must be alert and oriented.
    • Must not require IV therapy or other skilled nursing care.
    • Must be behaviorally appropriate for a group setting.

  • Choosing Change

    The Choosing Change program provides medication-assisted treatment and recovery support for patients who have opiate use disorder.

    Call the Behavioral Health Access Line at 1-800-846-1652 or talk to your doctor about how to get a referral.

  • Co-Applicant Governing Board

    The mission of the Health Care for the Homeless Co-Applicant Board is to oversee, guide and assist the program in its efforts to deliver high quality health care to a diverse and medically underserved community.

    Meetings

    All our meetings are open to the public. The Health Care for the Homeless Co-Applicant Board meets remotely the third Wednesday of every month from 11:00-12:30pm. These meetings focus on governance and oversight of the Health Care for the Homeless program and services, partners, processes, and policy. At this time all meetings are virtual. If you are interested in attending the virtual Health Care for the Homeless Co-Applicant Governing Board meeting, please reach out to linae.young@cchealth.org for the virtual meeting information.
